Runs when a fork PR build starts. Spends the approval when the trust policy asks for approval on new commits: the record goes back to pending, so the guard makes the next commit ask again.

It also asks GitHub again for PRs approved because their author can write to the repo. The guard can't make that call, so this is where we notice someone has lost access.
